When I set a skin and save it then reboot the Xbox the old skin is there instead of the new one.

You have unleashx correct? If so just follow this you have a program line code conflict in your config.xml in your E drive you will see two skin paths in you config.xml the lines your looking for are under the <Preferences> section of the xml, you should see one that looks like this
<Skin Path="E:\skins">Default </Skin>
and the other should look like this for example
<Skin Path="E:\skins">ProjectMayhem </Skin>
Just delete the line that has Default and save the xml and reboot
